Live Scan applicants must bring a completed  Live Scan Request Form to their scheduled appointment. If an applicant is to be Live Scanned for multiple agencies, a separate form is required for each. Fees are charged for each agency requesting the criminal records check.

Applicants must present to the Live Scan operator an unexpired CA Drivers License, DMV Identification Card or Out-of-State Drivers License.  For other acceptable forms of ID, please contact the Live Scan office.
