Wellness Program
The Optimal Health & Wellness Program is an integrated benefits program that encourages employees, their covered spouse or registered domestic partner and retirees to work with a team of health and wellness professionals to design a customized plan to improve their health and wellbeing.
 
The Program provides support by engaging the participant in their own “path” towards optimal health and wellness by providing a variety of resources and an integrated medical management model.

Are you a participant in one of the following plans:

  • Exclusive Care
  • Health Net
  • Kaiser Permanente

If so, this is your chance to get started on your way to earning your Silver (Level 1) by completing a Wellness Program Orientation.

Enroll in an on-demand Wellness Program Orientation >> 

 Silver (Level 1) requirements:

  1. Complete a Health Risk Assessment
    AND
  2. Complete a Biometric Screening
    AND
  3. Complete a Wellness Program Orientation (Live Lunch & Learn; Live webinar; On-demand webinar)

Incentive: $50.00 one time per Wellness Program year (November 1, 2012 – October 31, 2013)
