• Score Ability to set-up an Accountability Partner or work with a Wellness Coach from the Wellness Program

You must be sure to complete all data requested; failure to enter all information will disqualify you from fully participating in the OHW Program.  This information is necessary to properly risk stratify the Personal Wellness Profile Health Risk Assessment. * IMPORTANT: You must email the Wellness Program at wellness@rc-hr.com to obtain a lab slip to have your blood drawn to finish the enrollment process for 2011.
