Action Plan Options

If an eligible employee wishes to earn an achievement incentive, the employee is required to first complete the Health Risk Assessment and receive their Personal Wellness Profile®, complete a biometric screening, and complete an action plan yielding 12 or 24 points. 

This process is repeated annually
 
Action plans must be completed prior to October 31 of each year.
LDL ≤130 OR drop of 20 points
HDL ≥50 Women/≥40 Men OR increase of 5 points
Fasting Triglyceride ≤150 OR drop of 50 points
Fasting Blood Glucose ≤100 OR drop of 20 points
Hgb A1C ≤7% OR drop of 1 point
Blood Pressure (diastolic) ≤80 OR drop of 5 points (see note)
Waist Girth ≤35 Women/≤40 Men OR Waist Girth drop of 2 inches (see note)
2 Parameters met = 6 Points
4 Parameters met = 12 Points
 
Note: Requires original measurement by Physician or Wellness Team Member
  • Biometric screening: Level 2
    Maintain OR Achieve 3 or 5 of the following:
LDL ≤100 OR drop of 40 points
HDL ≥60 Women/≥50 Men OR increase of 10 points
Fasting Triglyceride ≤100 OR drop of 100 points
Fasting Blood Glucose ≤90 OR drop of 30 points
Hgb A1C ≤6% OR drop of 2 points
Blood Pressure (diastolic) ≤75 OR drop of 10 points (see note)
Waist Girth is ≤ half your height in inches OR Waist Girth drop of 4 inches (see note)
3 Parameters met = 12 Points
5 Parameters met = 24 Points

    You cannot accrue points for a Resiliency topic more than once a year.
    1 point per class; 6 points maximum.
  • Nutrition Classes
    Live Lunch & Learn; Live webinar; On-demand webinar
    You cannot accrue points for a Nutrition topic more than once a year.
    1 point per class; 24 points maximum.
  • Wellsource® – Health Challenges
    For your convenience, the wellness team has selected the most popular monthly health challenges (22 total). Each Challenge takes 3 to 4 weeks to complete. Each month (November - September) 2 health challenges will appear on the Wellness Website. You may choose to do both. Download the materials from the website. Wellness points will not be awarded for self-selected health challenges.

  • Telephonic coaching
    15-20 minutes sessions bi-weekly
    Available to ‘High’ Risk Individuals
    12 points maximum
