Monetary Incentives Model
The current monetary incentive model includes a criteria-based incentive program that is intended to catalyze participation in the program and specific health behaviors.
 
Four different levels of rewards are provided for meeting various numbers of the criteria:
  • Bronze (Level 1)
  • Silver (Level 2)
  • Gold (Level 3)
  • Platinum (Level 4) 
Eligibility is based on participation in one of the County of Riverside Medical Health Plans:
  • Exclusive Care
  • Kaiser Permanente
  • Health Net
Medical Insurance Coverage Waivers
The Gold and Platinum level rewards are regarded as additional contributions toward medical insurance coverage. If an employee achieves the Gold or Platinum level reward and waives medical insurance coverage at any time, the employee is no longer eligible to receive the reward.
 
Spouses and Registered Domestic Partners
Spouses and Registered Domestic Partners may participate in the program if they are on the Employee's County of Riverside Medical Health Plan and may only participate in the Bronze and Silver levels. 
 
Retirees
Eligible retirees are not entitled to the achievement incentives.
 
Levels of Rewards
 
Bronze (Level 1)
Complete a health risk assessment (HRA) – receive the “Personal Wellness Profile®”
Incentive: $25.00 one time per benefit year (January 1 – December 31)
 
Silver (Level 2)
Complete a HRA – receive the “Personal Wellness Profile®” and
Complete a biometric screening
Incentive: $50.00 one time per benefit year (January 1 – December 31)
 
Gold (Level 3)
Complete a HRA – receive the “Personal Wellness Profile®” and
Complete a biometric screening and
Complete an Action Plan yielding 12 points
Incentive: $30.00 contribution per month toward the cost of medical coverage sponsored by the County of Riverside beginning the following benefit year (January 1 – December 31)
 
Platinum (Level 4)
Complete a HRA – receive the “Personal Wellness Profile®” and
Complete a biometric screening and
Complete an Action Plan yielding 24 points
Incentive: $50.00 contribution per month toward the cost of medical coverage sponsored by the County of Riverside beginning the following benefit year (January 1 – December 31)
