How to File a Claim 
To file a claim call The Standard’s claim intake service center at (800) 378-2395.
You will need to provide the following information:

• Employer Name: The County of Riverside
• Group Policy Number: 641685
• Your name and Social Security number
• The date of the last day you were at work
• Nature of your claim/Medical information
• Your attending physician’s contact information (name, address, phone, and fax)
